The Role:

We are recruiting to Assistant Practitioner posts within our Transfer of Care Review Team.  Based in the community the Transfer of Care Review Team is part of Southwark’s Adult Hospital Discharge Team and supports service users who have left hospital but need further monitoring, assessment or review.  The team provides a multidisciplinary approach to assessing and supporting people and work using a strength-based approach focusing on people’s strengths, assets and abilities. This enables us to support individuals to make greater use of their community resources and to maximise their independence.

As part of the team you will be responsible for completing Care Act assessments and reviews to determine whether or not an individual is eligible for services.  This includes the undertaking of proportionate assessments and reviews, planning and proactively working with complex issues, family dynamics, crisis interventions, providing information and advice, transferring to other teams if required.
