Working in hospital discharge social work is all about being part of the multidisciplinary teams in the hospital and linking people to the services and things they need in the community.

A hospital admission is a major event in anyone’s life and our roles as social workers are to support people to get through that and to help them to achieve safe and timely returns to their lives outside of hospital. It can result in major changes and adjustments for the people we work with and their families and friends and each event is unique.

Every day at work is different with fresh challenges. We work as a team, giving and getting support from each other and in an environment that supports professional learning and development.
