The role:

We are recruiting Occupational Therapists for our Intermediate Care Southwark Reablement Service. Intermediate Care Southwark (ICS) is an integrated health and social care team delivering Urgent Community Response and Rehabilitation and Reablement services with Southwark residents. 

The purpose of our service is to provide home based Reablement care support and/or rehabilitation for Southwark residents 18 years and over in order to promote people’s strengths and independence, prevent unnecessary hospital admissions, facilitate timely hospital discharges and reduce reliance on long term care where possible.

The ICS Reablement focus includes optimising the person’s function, aids and confidence to support recovery: we work from a strength based position to build on existing skills and resources. 

Short term outcome focused interventions are delivered in people’s homes for a period of up to six weeks. This is achieved through multi-disciplinary approaches to help reach individual Reablement and/or therapy goals. 
The team offers targeted interventions from physiotherapists, occupational therapists, social workers, pharmacy staff and nurses, further supported by in-house therapy assistants and rehab support workers.

The service works alongside a Reablement care provider. The need for care is regularly reviewed within the period of Reablement. If, after the teams input, the person has ongoing care and support needs an assessment is completed to determine eligibility for ongoing services.

About you: 

We are looking for experienced, enthusiastic and motivated occupational therapists with well-developed community OT skills. You will be a good communicator and confident decision maker with an ability to work in a busy frontline service where you manage competing demands and prioritise your case load effectively. You can develop effective working relationships both inside and outside of your own profession and the service. 

Applicants should hold a Diploma or degree in Occupational Therapy, must be registered with HCPC, together with experience and knowledge of adult social care legislation. 

An enhanced Disclosure and Barring Service (DBS) check is required.
