Social Worker – Adult Social Care Mental Health – Move On Support Team
We are looking for an experienced Social Worker to join Southwark’s Move On Support Team within the Mental Health Adult Social Care Division. This is an exciting opportunity for an experience Social Worker to be part of an exciting, established and well-functioning team of Social Workers and Occupational Therapists.
You will provide comprehensive and holistic person-centred assessments and specialist interventions to enable people to maximise their wellbeing, independence, choice and control regarding their accommodation and support needs.
You will hold a small caseload to continue to develop your practice, and act as a role model and expert practitioner within the team. You will ensure the team’s work retains person centred and strengths based approaches, enhances collaboration with our partner agencies, and that we are meeting our legal obligations. You will build on the team’s ethos of reflective practice, learning and development.
About you:
The post holder is expected to be experienced Social Worker in Mental Health and will be expected to expand their specialist knowledge and skill relating to:
• Assessment and Support Planning
• Safeguarding / Risk Assessments
• Resources and procedures
• Identification of continuing healthcare and social care needs
• Identification of mental capacity, Safeguarding, DoLS and complex family issues
• Identification of motivation to change
• Sourcing mental health placement and seeking funding from placement and personalisation funding panel
• The post holder may need to travel outside of London to visit placements

We are the Mental Health Move On Support Team and the team is tasked with assessing, reviewing and supporting people in need of, or already placed in various mental health settings including supported living, residential and nursing care. The team ensures that placements are continuing to meet needs, are recovery focused and there are plans for eventual step down. The team supports and arranges discharge from hospital, secure units, high dependency units and prison. Our focus is supporting people to maximise their independence and choice, enabling them to live healthy, safe and fulfilling lives.
