Social Worker – Intermediate Care Southwark (Reablement) and Transfer of Care

We are recruiting to 3 x Social Worker posts

1x position is within the Intermediate Care Southwark – Urgent Response Team

The role provides an exciting opportunity to work in an urgent multi-disciplinary setting, with a focus on strengths based practice and promoting independence and recovery.

The main duties of the role include goal setting, completing urgent Care Act assessments and devising care and support plans, completing urgent reviews, urgent Mental Capacity Assessments+ Best Interests meetings, crisis intervention, signposting, providing advice and information and working with people with complex social needs.

There is also a Safeguarding function within the service, which includes completing initial fact finding and assessing and minimising risk

2x positions in the Transfer of Care Service

Southwark Transfer of Care (Hospital Discharge) Team North are seeking two qualified social worker for our busy team based at St Thomas Hospital. The team provides support for adult service users requiring assessment and provision of services to support safe and timely discharges from hospital.

Our social workers work closely with the hospitals multi-disciplinary teams to undertake a range of activities to facilitate discharges within the Discharge to Assess framework, including assessing for and arranging nursing and residential placements and generic packages of care. 

The team works with a high number of service users with cognitive impairments, therefore social workers regularly complete Mental Capacity Assessments and Best Interest Meetings / decisions, the team also receive Safeguarding referrals from the hospitals and carryout safeguarding enquiries as required.

About you:

You are enthusiastic and passionate about meeting the needs of the local community and are committed to working closely with those with care and support needs, their families and carers, alongside meeting the aims of the service. You are a resilient social worker, who is a confident decision maker and good communicator.

You can prioritise work effectively, manage competing demands and stay calm under pressure. You have experience of working collaboratively in multi-disciplinary settings and are able to develop effective working relationships both inside and outside of your own profession and the service.

You have the ability to problem solve, identify and manage risk and have an extensive knowledge, understanding and application of the legislation that relates to adults and older people including Care Act, Mental Capacity Act and Adults Safeguarding.

You hold a degree in social work and have completed the ASYE programme, are registered with Social Work England, with evidence of continuing professional development.
